We will also need to fill in your medical history form.<\/li>\n\n\n\n<li>Please arrive alone, without companions. People with mobility issues, the disabled, and children may arrive with a companion.<\/li>\n\n\n\n<li>It is recommended to wear a protective face mask and disinfect his\/her hands.<\/li>\n\n\n\n<li>You will receive test results by email. We will send you an SMS message containing a password to view your results.<\/li>\n<\/ul>\n\n\n\n<p><strong>Please bring the following:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A valid ID document. <\/li>\n\n\n\n<li>If the patient is your child, bring their birth certificate or any other form of ID. If you are bringing a child for an appointment who is not your family member, please bring a notary-certified power of attorney. <\/li>\n\n\n\n<li>If you have private insurance, please\nbring an insurance card.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\"><strong>Note:<\/strong> If you cannot attend your booked appointment, please let us know by replying to the reminder message with the word &#8220;NO&#8221;; you will receive this message prior to your appointment. Ask your doctor how to prepare for the ordered lab tests or <a href=\"https:\/\/nmc.lt\/en\/information-for-clients\/pasiruosimas-tyrimams\/\">click here&gt;&gt;<\/a> to find out. <\/p>\n\n\n\n<p>   <\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Arriving for surgery<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You should <strong>arrive<\/strong> 1 hour before the\nsurgery, unless otherwise stated.<\/li>\n\n\n\n<li>At the appointment desk, sign the\ncontract for the provision of healthcare services. We will fill in your&nbsp;medical\nhistory&nbsp;form.<\/li>\n\n\n\n<li>The staff will take you to the ward\nwhere you will be introduced to the pre-surgery information, complete the\nquestionnaire and sign the consent for surgery.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>Please bring the following items<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A valid ID document. <\/li>\n\n\n\n<li>If the patient is your child, please bring their birth certificate or any other form of ID.
