<p class="wp-block-paragraph">Rotavirus is a viral intestinal infection which causes vomiting, fever, diarrhoea and toxicity in the body. This infection has a seasonal pattern with a typical morbidity peak during the cold season. In Lithuania, there tends to be an increase in the morbidity rate between December and April.</p>



<p class="wp-block-paragraph">The group which most commonly suffers from infections caused by rotavirus is children under 5. Older people and people with a weakened immune system may also become infected with rotavirus, although this is rare.</p>



<p class="wp-block-paragraph"><strong>A source of the virus can be someone who is already infected by it. Most commonly infectious agents are spread by infected children and adults with a latent form of the infection</strong>. Rotavirus can spread when an individual touches an object contaminated with the virus and then puts their hand in or near their mouth.</p>



<p class="wp-block-paragraph">Initial symptoms of this disease can be observed within 24-72 hours after infection occurs. Light forms of rotavirus are rarely reported. The most common are mild forms. It has a rapid onset. The main early symptom is vomiting, which can last from 2 to 6 days. Following this, the patient starts to suffer from diarrhoea. The episodes of watery or semi-watery diarrhoea become frequent, occurring up to 20 times a day. Diarrhoea lasts longer than vomiting. At the onset of infection, a patient may experience a stomach ache. In children, rotavirus can cause symptoms of inflammation of the upper respiratory tract. Almost 90% of patients suffer from fever. Fever may vary greatly, starting with a low-febrile temperature and increasing until it becomes a very severe fever. It can last for 3-4 days. The virus can also disturb the digestion of dairy products. An infected person loses a lot of water and salts, which causes an electrolyte imbalance. Severe dehydration can be very dangerous for infants and children.</p>

<p class="wp-block-paragraph">Flu is one the most common kinds of epidemic. Long-term evidence has shown that a flu epidemic occurs every year and affects around&nbsp;<strong>5-10%</strong>&nbsp;of citizens.</p>



<p class="wp-block-paragraph">In Lithuania, flu season occurs in autumn and winter. It is difficult to predict exactly when a flu outbreak will occur, but evidence shows that the flu season tends to begin in October.&nbsp;<strong>According to the World Health Organisation (WHO), influenza is one of the most serious health issues in many countries</strong>. Influenza (also known as flu) is an acute viral respiratory infection, an airborne virus which easily spreads from person to person. Unlike other respiratory infections, flu is a very dangerous disease because of its potential complications.</p>



<p class="wp-block-paragraph"><strong>Vaccination is the most effective measure available to protect people from flu and the complications it can lead to.</strong>&nbsp;The reliability of modern medicine means the influenza vaccine is now totally safe. A yearly flu jab is recommended, because the genetic make-up of the influenza virus can change every year, so the flu vaccine composition must be reviewed. Revision of the composition helps to protect people from changing flu viruses more effectively. It is advisable to have a flu jab well ahead of the flu outbreak. The body needs time to respond to the vaccine and form an immunity for effective protection. The best time to get a flu jab is between September and November. After having a flu jab, the body will be protected against the influenza virus for the whole flu season.</p>



<p class="wp-block-paragraph">The WHO advises most people to get an influenza vaccination, but particularly pregnant women, children, people with chronic diseases and people aged 65 and over, who are all at the highest risk of contracting the virus.</p>

<p class="wp-block-paragraph"><strong>Cervical cancer is the second most common form cancer in women (after breast cancer).</strong>&nbsp;Cervical cancer takes some time to develop. The development period may take several months, or even several years. Furthermore, cervical cancer does not develop in a healthy mucous membrane (also known as inner lining). First, precancerous conditions occur in cervical cells. Timely screening can help identify such changes, manage them, and thus prevent the development of cancer.</p>



<p class="wp-block-paragraph">It is believed that human papillomaviruses (HPV) are responsible for 99.7% of cases of cervical cancer. HPV accounts for around 200 types of virus. The majority of these viruses cause warts and are not dangerous. Around 30 forms of this virus infect the inner lining of genital area, and around half cause genital warts (or condylomata acuminata, warts in pudental canals). Around 14 types of HPV can cause cancer. HPV types 16 and 18 are very dangerous forms of virus, and there is a high risk that cancer may develop from them. HPV mainly spreads through sexual activity.</p>



<p class="wp-block-paragraph"><strong>Protection against cervical cancer:</strong></p>



<ul class="wp-block-list"><li>primary prevention – preventive vaccination;</li><li>secondary prevention – for example, Papanicolaou smear, doctor’s examination, colonoscopy, etc.</li></ul>



<p class="wp-block-paragraph">The cervical cancer vaccine is a worthwhile step for almost every woman to take. The younger a woman is, the better the reaction from her immune system will be and the longer she will remain protected. Girls are advised to have this vaccine before reaching the age of 15 and starting sexual activity. However, the vaccine is also effective for elderly women, even if they already have a strain of HPV.</p>

<p class="wp-block-paragraph">Ticks form the majority of the animal group “arachnids”. A tick needs much more blood for nutrition than other parasites. If an attached tick remains unnoticed, it can suck blood for a period of 15 days. Hard ticks are known to be the most widespread type of ticks in Lithuania, and can be found almost in all parks and forests.</p>



<p class="wp-block-paragraph">When a tick sucks your blood, it transmits bacteria that cause severe disorders such as&nbsp;<strong>tick-borne encephalitis, Lyme disease, and Ehrlichiosis.</strong></p>



<p class="wp-block-paragraph">The tick-borne encephalitis represents the most dangerous disease. The&nbsp;death rate&nbsp;from the&nbsp;disease reaches 0.5 to 1% of the infected. One-third of people never&nbsp;make a&nbsp;full recovery after tick-borne encephalitis. The most common residual effects include psychoneurological symptoms such as inability to concentrate, sleeping disorders, chronic headache, increased sensitivity, behavioural changes, and more rarely, paralysis.</p>



<p class="wp-block-paragraph">A person who gets infected with tick-borne encephalitis virus, two or three days after a tick bite can develop flu-like symptoms such as a headache, fever, and dull pain in the body.&nbsp; These symptoms last for about a week. Afterwards, the majority of people feel better. In one out of three cases this virus can spread through the organism and even reach the brain lining, and the brain itself. The second stage of the tick-borne encephalitis can be marked with a fever, severe headache, vomiting, faintness, chilly sensation, stiff neck, and loss of consciousness.&nbsp; If the listed symptoms develop, immediately seek medical advice.</p>



<p class="wp-block-paragraph">There is no specific treatment, i.e. treatment against the infectious agent. A person can only receive treatment to relieve symptoms, i.e. a drug therapy for symptomatic relief.</p>



<p class="wp-block-paragraph">Vaccination is the most effective measure against a tick-borne encephalitis. The effectiveness of the vaccine against tick-borne encephalitis reaches 98%.</p>



<p class="wp-block-paragraph"><strong>Standard vaccination plan:</strong></p>



<ul class="wp-block-list"><li>the first dose;</li><li>the second dose (1-3 months after the first vaccination);</li><li>third dose (5-12 months after the second dose).</li></ul>

<p class="wp-block-paragraph">If you are planning to travel, it is recommended to learn about the risks of contagious diseases and possible preventive measures. To get your immunity formed, you need to wait for some time after the vaccination. This period depends on the type of disease you are vaccinated against, the number of required doses and whether a person has been previously vaccinated from it.&nbsp; So you should visit your doctor at least 4-6 weeks before you depart.</p>



<p class="wp-block-paragraph">After a family doctor evaluates all risk factors of the trip and the traveller’s health status, he will draw an optimal plan for protection. All administered vaccines will be recorded in the international immunisation passport.</p>



<p class="wp-block-paragraph">The following factors must be taken into consideration for vaccination:</p>

<p class="wp-block-paragraph"><strong>What are
the most common examinations performed when diagnosing disorders in digestive
organs?</strong></p>



<p class="wp-block-paragraph">Before
planning any examinations and tests, it is important to talk to the patient,
find out about current symptoms and his medical case history. Based on that, a
patient will receive an individualised plan of laboratory and instrumental
tests. The main instrumental examinations applied for diagnostics of intestinal
disorders include pelvic ultrasound, upper gastrointestinal endoscopy, and
endoscopic examination of the large bowel&nbsp;(also known as colonoscopy).<strong></strong></p>



<p class="wp-block-paragraph"><strong>What is
an endoscopic examination of the large bowel and colonoscopy?</strong></p>



<p class="wp-block-paragraph">The upper
gastrointestinal examination is a procedure performed using a special endoscope
to examine the oesophagus, stomach, and the upper small bowel called duodenum.
Meanwhile, a colonoscopy is a procedure that enables an examiner to evaluate
the inside of the colon (large bowel) with an endoscope (a special flexible
tube).</p>



<p class="wp-block-paragraph"><strong>Why is a
colonoscopy performed?</strong></p>



<p class="wp-block-paragraph">The upper
gastrointestinal endoscopic examination must be done if you have pain or any
type of discomfort in the upper part of the stomach. In case previous treatments
bring no results. Or in case of any &#8220;dangerous&#8221; symptoms, such as, newly
occurred symptoms in people of 50-55 years or older; if any of your relatives
had stomach or oesophagus cancer; weight loss, bleeding, difficulties with
swallowing or painful swallowing, iron-deficiency anemia of unknown origin,
chronic vomiting, palpated mass in the stomach or enlarged lymph nodes,
jaundice. This type of examination is also recommended when a patient
experiences difficulty in swallowing or endures painful swallowing, has
recurring or long-term gastroesophageal reflux disease, vomiting of unknown
origin, an iron deficit, or abnormal tissues diagnosed by other radiological
examinations. The examination can also be performed as part of a treatment.</p>



<p class="wp-block-paragraph">A
colonoscopy can be performed as part of a routine screening (Programme for
Early Detection of Large Bowel Cancer), as a follow-up examination for patients
with inflammatory bowel diseases, after resected polyps and large bowel cancer,
or for the assessment of expansion of inflammatory bowel diseases.&nbsp; This examination can also be useful in
patients with active or chronic bleeding (unknown reason of iron-deficit
anaemia), chronic complaints of pelvic pain for unknown reasons (for example,
chronic diarrhoea), sudden changes in the nature of defecation (constipation
along with other dangerous signals like weight loss, anaemia, blood in faeces)
or abnormal tissues diagnosed by other radiological examinations.</p>



<p class="wp-block-paragraph">The upper
gastrointestinal endoscopic examination and a colonoscopy can be performed as
part of a treatment.</p>



<p class="wp-block-paragraph"><strong>What is
the course of these examinations and how to prepare for them?</strong></p>



<p class="wp-block-paragraph">Before
performing any examinations, each patient receives information on the course of
the procedure, about the possible discomfort she can feel and the actions that
can be taken to reduce discomfort. The patient&#8217;s confidence in his doctor and the
examination technique to be performed by a qualified specialist are very
important for the success of the procedure. This is a very specific
examination, which requires an experienced doctor. The more accurately the
examination is performed, the better the evaluation of pathological tissues.
When a patient is being prepared for these examinations, he receives all the information
about possible complications and signs a consent form.&nbsp; If a patient refuses the examination, it is
not performed.</p>



<p class="wp-block-paragraph">You must
not eat at least 6 hours before your procedure. For people with prolonged bowel
evacuation it is recommended to avoid eating for even longer periods. In the
morning, you can take your medications (to reduce blood pressure, for example)
with a little water. If a patient has diabetes, an examination should be
performed in the morning. Otherwise, you will need to adjust the time of the drugs
prescription. If a patient takes medicines affecting the blood´s clotting
ability (aspirin or clopidogrel), it is necessary to evaluate the risks of
thrombosis. It is recommended to avoid taking these drugs for at least 5 days
before a colonoscopy.</p>



<p class="wp-block-paragraph">The preparation
of your bowel for a colonoscopy plays an important role. Usually, the preparation
begins a day before the procedure. You will need to drink special liquids to clean
your colon. If a colonoscopy is scheduled in the afternoon, it is recommended
to drink half of the liquid in the evening, while the other half early in the
morning (the day of the exam).</p>



<p class="wp-block-paragraph"><strong>Many
patients dread endoscopic examinations because of discomfort. Are there any
ways to avoid it?</strong></p>



<p class="wp-block-paragraph">Although
endoscopic examinations may seem unpleasant, they are necessary to diagnose the
disease in early stages and apply successful treatment. Also, the discomfort
experienced during the procedure can nowadays be suppressed with the help of
medicines.</p>



<p class="wp-block-paragraph">Regecting
a foreign body is a natural human reflex. The degree of sensitiveness varies
among people, as does the pain threshold.&nbsp;
Anyway, a patient receives a local spray&nbsp;anaesthetic
(lidocaine)&nbsp;into the&nbsp;throat before the upper GI endoscopic exam. Some
patients unreasonably dread these exams, but endure them quite easily. While
other patients aren&#8217;t afraid of the exam at all, follow the instructions, but
their vomiting reflex is so strong that it makes the procedure more difficult
to perform. Such patients receive a short-term intravenous anaesthesia. This
general anaesthesia lasts only a few minutes. Doctors use special medicines
that have a short-term effect with no adverse reactions.</p>



<p class="wp-block-paragraph">The pain
threshold to a colonoscopy depends on the patient bowel&#8217;s anatomy. The bowels
may be bent or twisted thus it could be difficult to access them. The movement
of the endoscope through the bowels become complicated due to clots formed
after pelvic surgeries. At Northway Medical and Surgical Centre, this
examination in all cases is performed under a general intervenous anaesthesia,
so the patient feels nothing.</p>



<p class="wp-block-paragraph"><strong>What kind
of abnormalities can be detected during these examinations?</strong></p>



<p class="wp-block-paragraph">The upper
gastrointestinal endoscopic exam can help diagnose both malignant and
non-malignant diseases or make sure no pathology is present. Non-malignant
diseases include oesophageal erosions, stomach erosions, duodenal erosions,
ulcers, and inflammatory changes of various origins in the stomach´s mucose
membrane. It also helps to diagnose malignant, i.e. cancerous diseases. </p>
