<p>When an endocrine gland produces an excess of hormones, it is called hyperfunction. On the other hand, a decrease in hormone production is referred to as hypofunction. Hormonal disorders can lead to various health conditions. For example, the pancreas produces insulin, a hormone that helps regulate blood sugar levels. Insufficient insulin production can disrupt the transport of glucose into cells, leading to elevated blood sugar levels and the development of diabetes mellitus. Dysfunction of the pineal gland, which synthesizes the hormone melatonin responsible for regulating the biological clock (circadian rhythms), can cause sleep disturbances, loss of energy, and decreased work performance. <p>Thyroid dysfunction can cause symptoms such as anxiety, mood swings, frequent fatigue, rapid heartbeat, and insomnia. While thyroid dysfunction is more common in women, it can also affect men. Risk factors for thyroid diseases include age (the risk increases with age), iodine deficiency (or excess) in food, and exposure to ionizing radiation (depending on the dose and duration of exposure).</p>



<p>There are two main types of thyroid disease. The first type comprises disorders caused by inadequate hormone production, which is the main function of the thyroid gland. Insufficient hormone production leads to general weakness, slow thinking, easy fatigability, dry skin, hair loss, facial swelling (particularly around the eyes), leg swelling, loss of appetite, weight gain, irregular periods, sensitivity to cold, and a desire to keep warm. Excessive hormone production leads to insomnia, mood swings, anxiety, hyperactivity, sweating, thirst, muscle weakness, increased appetite, weight loss, sensitivity to heat, a strong desire to cool down, possible diarrhea, and changes in menstrual cycles. Both hormone deficiency and excess lead to poor health.</p>



<p>The second type involves changes that affect the thyroid tissue. Typically, these changes manifest as neck pain, particularly felt at night or when swallowing food. Changes affecting the thyroid can cause voice changes, difficulty breathing, coughing, and fever.</p>



<p>Goiter is the most common thyroid-related endocrine disorder. There are two types of goiters: diffuse goiter (enlargement of the entire thyroid gland) and nodular goiter (development of nodules in the thyroid tissue). Enlargement of the thyroid gland is the main symptom of goiter. There are three grades of thyroid enlargement. Grade 1 is characterized by a normal-sized thyroid gland with a visible nodule or lump only when tilting the head back. In grade 2, the lump is visible when the head is in a normal position. In grade 3, the lump is clearly visible. Goiter typically presents as a swollen neck, discomfort when swallowing, difficulty breathing, fatigue, hoarseness, rapid or irregular heartbeat, weight loss, and nervousness.</p>



<h2 class="wp-block-heading"><strong>Treatment of endocrine disorders</strong></h2>



<p>Thyroid dysfunction is typically treated with medications. Hormone replacement therapy, in the form of thyroid hormone pills, is prescribed when the thyroid gland slows down and produces insufficient hormones (hypothyroidism). In cases of excessive hormone production (hyperthyroidism), treatment involves medications known as anti-thyroid drugs, which help reduce the production of excess hormones by the thyroid. When medical therapy is ineffective, hyperthyroidism may be treated with radioactive iodine or surgery, which involves the partial or complete removal of the thyroid gland responsible for the excessive hormone production.</p>



<p>If you have been diagnosed with an underactive thyroid or hormone deficiency (which can occur due to inflammation, surgical removal of the thyroid gland, or a part of it), you will need to take medication for the rest of your life. In the case of excess thyroid hormone levels detected through blood tests, your doctor may prescribe long-term therapy with anti-thyroid drugs. Over time, the doctor will gradually decrease the dosage until it reaches a minimal level or even discontinue the medication. However, if the disease recurs despite medical therapy, your doctor may recommend surgery or radioactive iodine treatment.</p>

<p>A rapid and accurate urease breath test is performed to diagnose H. pylori (Helicobacter pylori) infection. Helicobacter pylori settles in the lining of the stomach, causing inflammation (gastritis). In many cases, this infection does not cause symptoms, but if the inflammation becomes acute, it can lead to indigestion, burning pain in the upper abdomen, and nausea. In rare cases, it can cause vomiting, bloating, or loss of appetite. The urease breath test can also be used to check the effectiveness of prescribed treatment and confirm its success. The IgG blood test, with qualitative and quantitative range, is effective in diagnosing H. pylori, which is one of the most widespread infectious diseases worldwide.</p>



<p>Endoscopic examinations play a crucial role in diagnosing even minor changes in the condition of the esophagus, stomach, duodenum, and large bowel, thereby helping to prevent insidious diseases. Endoscopy is a procedure that allows doctors to visualize the inside of a person&#8217;s body using a specialized instrument called an endoscope. Gastrointestinal endoscopy, or endoscopic examination of the digestive tract, provides a high accuracy rate of 98-100%.</p>



<p>Gastroscopy, also known as upper gastrointestinal endoscopy, is used to examine the spleen, stomach, and a portion of the duodenum. It is recommended in cases of pain or discomfort in the upper part of the stomach, lack of improvement with previous treatments, &#8220;dangerous&#8221; symptoms in individuals aged 50-55 or older, family history of stomach or esophageal cancer, unexplained weight loss, bleeding, difficulty or painful swallowing, iron-deficiency anemia of unknown origin, chronic vomiting, palpable mass in the stomach or enlarged lymph nodes, and jaundice. Gastroscopy is also recommended for patients with recurring or long-term gastroesophageal reflux disease, unexplained nausea and vomiting, and to differentiate between functional disorders (such as functional dyspepsia) and symptoms caused by possible organic neoplastic diseases. It is used for active and chronic bleeding, iron deficiency, when other radiological examinations have detected pathological formations that require further clarification, and for endoscopic follow-up of pre-cancerous or neoplastic conditions, as well as for monitoring post-endoscopic or surgical treatments.</p>



<p>Gastroscopy aids in diagnosing both malignant and non-malignant diseases, as well as ensuring the absence of pathology.</p>



<p>Colonoscopy is a procedure in which a doctor uses a colonoscope to examine the lower part of the gastrointestinal tract (colon). Colonoscopy can be performed as part of routine screening under the Program for Early Detection of Large Bowel Cancer. Adults aged 50-74 are advised to undergo fecal occult blood tests. If the results are positive, it is strongly recommended to have a colonoscopy. To arrange this examination, you should consult with your GP. Colorectal cancer is one of the most prevalent diseases in Lithuania, with approximately 1,500 new cases detected each year, and the number is increasing annually. Detecting colorectal cancer at an early stage provides the greatest chance for a cure. Unfortunately, patients may not experience any signs of the disease for an extended period. For this reason, regular fecal occult blood tests are important for predicting early-stage colorectal cancer. Colonoscopy, including biopsy when necessary, can confirm or rule out colorectal cancer. The importance of regular examinations cannot be overstated, as timely diagnosis leads to effective treatment.</p>



<p>Colonoscopy can also be used as a follow-up test for inflammatory bowel disease, after the removal of polyps and colorectal cancer, and to assess the spread of inflammatory bowel disease. It may also be recommended in cases of active or chronic bleeding of unknown cause, chronic pelvic pain, sudden changes in bowel habits (such as constipation) accompanied by other concerning signals like weight loss, anemia, or blood in the stool, and for the evaluation of abnormal tissues diagnosed through other radiological examinations.</p>



<p>Typically, colonoscopy helps diagnose polyps, diverticulum, large bowel cancer, and inflammatory bowel diseases. In addition to visual examination, the doctor may take a tissue sample (biopsy) for further analysis by a pathologist.</p>



<p>Both upper gastrointestinal endoscopic examination and colonoscopy can also be used as therapeutic procedures. The most common endoscopic treatment methods include polyp resection in the large bowel, removal of stomach polyps, pre-cancerous tissues, and early stomach cancer, dilation of narrowed lumens, stent placement, control of bleeding, and removal of foreign bodies from the esophagus.</p>



<h2 class="wp-block-heading"><strong>When should you see a gastroenterologist?</strong></h2>



<p>If you frequently experience bloating and pain, it is advisable to schedule an appointment with a gastroenterologist. If these symptoms occur after almost every meal and are accompanied by nausea or painful bowel movements, it is recommended to seek an examination from a gastroenterologist. Additionally, severe symptoms such as vomiting, weight loss, and loss of appetite should not be ignored and require immediate attention from a gastroenterologist.</p>



<p>Certain symptoms should never be ignored, including constipation (fewer than three bowel movements per week), acute or chronic diarrhea (three or more bowel movements per day with watery, liquid stools), and the presence of blood in the feces. Bleeding is commonly caused by conditions such as hemorrhoids, anal fistulas, intestinal infections, or inflammatory bowel disease. However, it can also be a warning sign of malignant disease, particularly in individuals over the age of 50 or those with risk factors such as long-term smoking, a family history of cancer, or a high intake of red meat.</p>



<p>Frequent reflux (indigestion) and frequent use of antacids can be another reason to consult a gastroenterologist. Frequent reflux may indicate conditions such as gastritis, stomach ulcers, dysbiosis, esophageal thrush, or gastroesophageal hernia. If you experience symptoms such as difficulty swallowing, the sensation of food getting stuck, or painful swallowing, it is important to contact a gastroenterologist promptly as they can be signs of a malignant disease.</p>



<p>Persistent esophageal pain, difficulty swallowing, or worsening of swallowing over time should not be ignored. If you experience these symptoms, it is recommended to consult a gastroenterologist and undergo an endoscopic examination of the esophagus and stomach (gastroscopy) to determine the underlying cause.</p>

Additional tests may be ordered by the doctor for an accurate diagnosis. Ultrasound scans also serve as a reliable tool in diagnosing endometrial polyps, which are changes in the uterine lining. It is recommended to have this test performed after menstruation when the endometrium is at its thinnest, allowing for the detection and diagnosis of even minor changes.</p>



<p>The ultrasound specialists at Northway Medical Centres would like to remind patients that routine check-ups should include an ultrasound scan once every two years, even if there are no complaints or diagnosed conditions. In the case of detecting abnormalities or changes during the examination, it should be repeated every 6-12 months depending on the nature of the findings.</p>



<p>An <strong>endoscopy </strong>is a diagnostic tool commonly used to diagnose disorders of the stomach, gastrointestinal tract, and large bowel. It involves the use of a flexible optical device to examine the lining of internal organs, detect abnormalities in the mucous lining, and assess its condition. If necessary, a doctor may take a biopsy during the endoscopic exam to obtain results indicating the presence or absence of cancer. Endoscopy can also be used to stop bleeding, remove polyps, or foreign bodies. Additionally, an endoscopic exam can serve as a tool to evaluate abnormalities during the course of treatment.</p>



<p>At Northway Medical Centre, our specialists perform various types of endoscopic exams, including upper gastrointestinal endoscopy (or upper endoscopy), colonoscopy of the large bowel, and others.</p>



<p>Upper gastrointestinal endoscopy (or gastroscopy) is recommended for individuals experiencing symptoms such as stomach pain or discomfort, unexplained nausea and vomiting, digestive disorders, bloating, bleeding, anemia, weight loss, or loss of appetite.</p>



<p>A Colonoscopy may be indicated for patients with symptoms such as rectal bleeding, bloody or non-bloody diarrhea, anemia, bowel evacuation disorders (such as constipation or recurring diarrhea), blood in stool, a feeling of incomplete bowel movement, or weight loss. In the event of detecting any pathological abnormalities (such as inflammation, polyps, tumors, etc.), the doctor may take a biopsy and conduct a microscopic examination of the tissue. This examination may also be performed to diagnose bowel cancer.</p>



<h2 class="wp-block-heading"><strong>How to prepare for the examinations</strong></h2>



<p>Effective patient preparation is crucial for ensuring the quality of the examination outcome. Failure to follow the provided preparation instructions may result in the postponement of the examination, as the doctor will be unable to perform it properly. To ensure the most accurate outcome and avoid the need for repeat procedures, we have compiled a list of instructions prepared by our medical specialists. By following these instructions, you can be confident in obtaining the best possible results.</p>



<h3 class="wp-block-heading">How to prepare for an X-ray</h3>



<ul class="wp-block-list">
<li>There is no need for special preparation for an X-ray examination. However, during an X-ray, patients may be asked to remove certain clothing or jewellery that could obstruct normal visualization.</li>
</ul>



<h3 class="wp-block-heading">How to prepare for an ultrasound scan</h3>



<ul class="wp-block-list">
<li><strong>If you are an adult undergoing an ultrasound scan</strong>, it is recommended that you refrain from eating or drinking for 6 hours prior to the procedure. The evening before the exam, have a light meal and avoid consuming foods known to cause flatulence, such as pickled foods or foods containing flour. For children, eating should be avoided for 4 hours before the procedure, although they can drink up to 100 ml of still water up to 2 hours prior.</li>



<li><strong>For a kidney and bladder ultrasound</strong>, it is important to drink approximately 1.5 litres of still water 2 hours before the procedure and arrive with a full bladder. If maintaining a full bladder is challenging, you may choose to arrive at the clinic early (30 minutes prior) to consume the water, but make sure to inform your doctor.</li>



<li><strong>For pelvic ultrasounds or kidney and bladder ultrasounds</strong>, fasting is required for 6 hours before the procedure. However, you can drink up to 1.5 litres of still water 1-2 hours prior and should arrive with a full bladder for the examination.</li>



<li><strong>For a renal artery ultrasound,</strong> fasting from both food and drink is required for 6 hours before the procedure. The evening before the exam, have a light meal and avoid consuming foods known to cause flatulence.</li>



<li><strong>Prior to a transrectal ultrasound, </strong>it is advisable to drink fluids and maintain a full bladder.</li>



<li><strong>If you are preparing for a gynaecological ultrasound</strong> scan performed through the abdominal wall, it is recommended to consume liquids and ensure that your bladder is full.</li>
</ul>



<h3 class="wp-block-heading">How to prepare for endoscopic examinations</h3>



<ul class="wp-block-list">
<li><strong>For a gastrointestinal endoscopy (fibrogastroscopy)</strong>, it is important not to eat or drink anything for 7–8 hours prior to the examination. If the procedure will be performed under sedation, it is necessary to refrain from eating and drinking throughout the entire day. If you are taking medication for blood pressure control, please take them in the morning with some water.</li>



<li><strong>In preparation for proctology exams such as rectoscopy or anoscopy</strong>, it is necessary to use 4 micro-enemas before the examination. Two micro-enemas should be used in the evening, and the remaining two should be used 2 hours before the exam. Micro-enemas can be purchased at a pharmacy. Additionally, it is important to abstain from eating or drinking (except water) for 6 hours before the exam.</li>



<li><strong>When undergoing a colonoscopy,</strong> proper bowel preparation is crucial for optimal procedure outcomes, as emphasized by medical specialists. Incomplete bowel cleansing may hinder the detection of abnormalities. 
<ul class="wp-block-list">
<li>It is imperative to follow the doctor&#8217;s instructions and adhere to a special diet for 3-7 days prior to the procedure. This preparation is necessary to ensure accurate results, diagnose pre-cancerous polyps, remove them, and prevent the development of bowel cancer. </li>



<li>On the day preceding the procedure, a special formula must be consumed to clear the bowel. If the colonoscopy is scheduled in the morning (8.00-12.00 a.m.), the preparation formula should be consumed on the previous night. For afternoon appointments, it is recommended to consume half of the formula in the evening and the other half in the early morning of the exam day. In the case of an evening colonoscopy, the entire preparation formula can be consumed in the morning of the exam day. It is essential to complete the consumption of the formula at least 6 hours before the examination. In instances of constipation, the doctor may suggest performing the bowel preparation procedure a few days prior to the exam. </li>



<li>For 2-3 days before the procedure, it is advisable to consume low-fiber foods. It is recommended to avoid seeds, nuts, whole grains, cereals, red/purple-colored food, and drinks. On the day before the procedure, a light breakfast and clear fluids are permitted. Eating or drinking is not allowed on the day of the procedure. If you need to take any medication on the exam day, you can do so in the morning with some water.</li>
</ul>

<h2 class="wp-block-heading"><strong>What is MRI?</strong></h2>



<p>Magnetic Resonance Imaging (MRI, MR) is one of the most advanced, safe, and comprehensive diagnostic methods. It is a non-invasive procedure that uses a strong magnetic field and radio waves to produce high-resolution images of internal organs and soft tissues. No harmful ionising radiation is emitted during the procedure. Magnetic resonance imaging examines various areas and organ systems of the body. Namely the musculoskeletal and joint systems, head, spine, abdominal organs, kidneys, urinary tract, and the small pelvis.</p>



<h2 class="wp-block-heading"><strong>Features and benefits of MRI</strong></h2>



<p>One of the most advanced diagnostic systems on the market, the Philips MR500 1.5T, is characterized by its accuracy, testing speed, and exceptional comfort. At our centers in <strong>Vilnius </strong>and <strong>Klaipėda</strong>, magnetic resonance imaging (2D and 3D) is performed up to 50% faster, and an integrated special audiovisual system allows the patient to fully relax during the examination.</p>

<h2 class="wp-block-heading"><strong>MRI examination safety</strong></h2>



<p>Magnetic resonance imaging is a painless and non-invasive examination that does not cause any known associated side effects.</p>



<p>Before undergoing an MRI examination, please consult your treating physician or a staff member of the diagnostic center, specifically a radiologic technologist, if you are pregnant or suspect that you might be, or if you have recently had surgery.</p>



<p><strong>MRI examination cannot be performed if the patient has the following items in his/her body:</strong></p>



<ul class="wp-block-list">
<li>Heart pacemaker, implantable cardioverter defibrillator, or artificial heart valves;</li>



<li>Metal clips or metal plates in the skull after neurosurgery procedures;</li>



<li>Vascular, eye and ear implants;</li>



<li>Metal implants, bullet residues that contain iron, nickel or cobalt;</li>



<li>Drug infusion pumps (morphine, insulin delivery, and other drugs);</li>



<li>Other foreign bodies, such as shrapnel, other unspecified electronic, mechanical, or magnetic implants;</li>



<li>Pregnancy as relative contraindication (the first 12 weeks);</li>



<li>Women with a contraceptive coil should consult a gynaecologist before the test to ensure that the coil is compatible with the MRI machine.</li>
</ul>



<p>An MRI scan may be performed if you have fillings or other dental implants.</p>



<h2 class="wp-block-heading"><strong>MRI procedure steps</strong></h2>



<p>You will be asked to lie down on a special mattress on the machine&#8217;s table. A special button will be placed in your hand, which you can press to summon a member of staff. During the test, you will be guided by a radiologic technologist. If you have a fear of enclosed spaces (claustrophobia), you can stop the test by pressing a special button. To obtain high-quality images, you should lie still during the examination. You will hear various sounds during the test, but don&#8217;t be afraid; these are the normal sounds of the device.</p>



<h2 class="wp-block-heading"><strong>Getting prepared for an MRI exam</strong></h2>



<p>Unless otherwise stated, an MRI scan requires no special preparation. You can eat and drink normally and take your regular medication before the test. Before your procedure, you will be asked to fill in a special questionnaire to check for any contraindications to the test. It is also important to remove any metal objects (jewellery, belts, etc.) before the test, and you may need to change out of the clothes you are wearing and into disposable clothing, which is available for purchase at our Centre.</p>
