Intrauterine insemination (IUI) is performed during a natural cycle at the time of ovulation or by stimulating ovulation with medications. During IUI, specially prepared sperm cells are placed directly into the woman's uterus using a specialized catheter. This method is appealing because it does not require surgical intervention or anesthesia. It is a straightforward and painless procedure.

In vitro fertilization (IVF) (outside the woman's body) is a method in which eggs are collected from a woman's body after being stimulated with medication and are combined with purified sperm from a man in a laboratory. This process takes place in a test tube. It is very similar to natural conception, except that the embryos are grown in an incubator and then transferred to the uterus. To achieve the best results, Northway Fertility Centres use Esco Miri® TL embryo incubators, which utilize a time-lapse (non-invasive periodic imaging) method to monitor embryo development at any moment, allowing only the best embryos to be selected for transfer to the uterus.
As in natural process, reproductive cells are necessary during the treatment. The eggs are retrieved (by aspirating them from the follicles) during the ovarian puncture, while the sperm is collected from a semen sample. If there are no sperm cells in the semen and azoospermia is diagnosed, in some cases sperm can be obtained from the testes during the TESA procedure. The reproductive cells are not artificially created; they are natural and belong to the couple undergoing treatment.
In order for an egg to be fertilized, it is sufficient to place it in an environment containing hundreds of thousands of sperm cells. In other words, if the sperm cannot reach the egg or if the surrounding conditions are not suitable for them within the woman's body, ideal conditions for the fusion of the cells—the meeting of sperm and egg—are created in the laboratory. Fertilization occurs when at least one of the sperm cells enters the cytoplasm of the egg and fertilizes the nucleus. A single drop of suitable medium, the right temperature, and other parameters that mimic a woman's body are all that is needed for fertilization to take place.
ICSI (intracytoplasmic sperm injection) primarily addresses male infertility issues, such as poor sperm motility, reduced sperm count, and the inability to penetrate the egg. In such cases, the embryologist must select morphologically normal, the most motile, and physiologically mature sperm cells.
Using this method, the woman’s eggs are placed in a test tube, and then a single sperm cell is directly injected into each egg. This procedure requires a high level of skill and responsibility from the embryologist to ensure that the most suitable sperm is selected. Using this assisted reproductive technology, the likelihood of conception is higher compared to traditional IVF methods, particularly in cases of male factor infertility and unexplained infertility.
If there are no sperm cells in the semen sample, they can be directly extracted from the testes during the TESA procedure. This is a microsurgical technique in which seminiferous tubules are taken from the testes to search for sperm cells.
